Our Assertive Community Treatment Team (ACTT) delivers intensive mental health care to adults living with severe and persistent mental illness—meeting clients where they are to reduce hospitalizations, improve stability, and support recovery.

What You’ll Do

Nursing & Medical Support

• Coordinate care with providers, nurses, and healthcare partners
• Support medication compliance and medication management
• Monitor medication inventory, refills, and supplies
• Record vitals and maintain accurate documentation
• Educate clients and staff on medications and health needs

Community-Based Client Support

• Meet clients in homes and community settings
• Transport clients to medical appointments and community resources
• Help reduce barriers to treatment and improve continuity of care

Crisis Response & On-Call Rotation

• Participate in rotating crisis phone coverage
• Assist with crisis planning and hospital diversion efforts
• Support commitment procedures when clinically appropriate

Qualifications

✔ Active North Carolina LPN license OR Certified Medical Assistant (CMA) certification
✔ Valid driver’s license with ability to travel locally
✔ Reliable transportation
✔ Comfortable working independently in community settings

Preferred

✔ Behavioral health experience
✔ Community health or ACTT experience
✔ Strong communication and teamwork skills
